Required Forms for Enrollment

All families need to complete the following forms before starting at A+ Learning Center:

2025 Parent Handbook

Contains all program policies, guidelines, and helpful information for families.

Handbook Acknowledgment

Confirms that you have read and understood the Parent Handbook.

Enrollment Form

Provides essential information about your child and family.

Certificate of Immunization

Required by Washington State law to ensure your child’s health and safety.

DCYF Payment Agreement

Details your tuition agreement and payment plan

DCYF Permission Form

Grants permission for activities and care as required by the Department of Children, Youth, and Families.

Media Release Form

Allows us to share photos or videos of your child participating in programs (optional).
